When accidents or incidents occur, there are consequences of various losses including personal injury, working time, financial losses, costs for legal support and potential claims, credibility of the company, the worst outcome being loss of business or prosecution leading to closure of the company and loss of lives. 

It is imperative that accidents and incidents are prevented in the first instance, but where they occur adequate reporting must take place and thorough investigation to find the causes and stop reoccurrence or more severe consequences in the future.

When incidents are investigated, the emphasis should be concentrated on finding the immediate, underlying and root causes of the incident so you can prevent the event from happening again. The purpose is to find facts that can lead to corrective actions, to stop it happening again, or in a different area of the company but not to find fault. Always look for deeper causes. 

Accident or incident investigations can often be the first document asked for by the authorities or when a claim is being pursued.

  • Identifying risk control measures
  • The action plan and its implementation
